4OVER4 asks for 1/8 in of bleed on standard print and 1/4 in on large format. This page uses the standard figure because the configurator marks this product as standard print.
A door hanger is a rack card with a hole in it. The exact die varies, so keep the top 2 in clear and confirm the die before the run.
Treat the hole as a hard exclusion. Nothing readable within half an inch of it, and nothing at all in the slit that runs from the hole to the top edge.

A door hanger is a rack card with a hole in it, and the hole is where designs go wrong. Pick a size and see the die drawn to scale, with the area the hole eats.
The standard door hanger is 4.25 x 11 inches. 4OVER4 also prints 4.25 x 14, 3.5 x 8.5 and 3.5 x 11 inches. The hanger hole is about 1.5 inches across, so keep the top two inches clear.
The hole is not decoration
A door hanger hangs from a handle, so a hole is die-cut near the top, usually about 1.5 inches across with its centre about 1.5 inches down. Everything in that circle is gone, and the ring of card around it is structural: put a logo there and it tears off the first windy afternoon.

Which size for which job
4.25 x 11 in is the standard and the one most people mean. 4.25 x 14 in gives you a tear-off coupon at the bottom. 3.5 x 8.5 in and 3.5 x 11 in are the slim versions, cheaper to print and post, and they still fit a standard handle.
All four are read at arm's length on a doorstep, so 300 DPI at the finished size is right and the headline wants to be big.
