What has to appear on a printed sign up form or counter card?

Enough for someone to decide before they hand anything over: who the controller is, what you will use the address for, roughly how long you keep it, that they can withdraw or object, and where the full notice lives. What does not work is a card that only says "we respect your privacy", because it names no purpose and no route out, which are the two things a person actually needs.
